Every third weekend, two x 8-hour shifts with 1 day off the week before or after.
Every three weeks, one 4-hour evening shift

Guthrie PGY1 pharmacy residents are required to be licensed in the state of 宾西法尼亚.  This requires passing NAPLEX and PA MPJE.
居民 are required to be licensed in the state of 宾西法尼亚 within 120 days after start of residency. Failure to obtain licensure may result in dismissal from the program.

完成标准
Prior to receiving a certificate of completion, the resident must complete all major program requirements including successful completion of the following:

  • Completion of an exit interview with the Program Director or designee
  • Completion of required learning experience, preceptor and self-assessments
  • Completion of one (1) major presentation at an appropriate forum as determined by the Program Director
  • Submission of a manuscript suitable for publication
  • Completion of a major resident project.
  • Completion of a medication use evaluation (MUE) (see below)
  • Achievement of “Achieved” or greater for at least 80% of all learning objectives 
  • Scoring of “Achieved for Residency” for the following learning objectives
    • R1.1.1. (Applying) Interact effectively with health care teams to manage patients’ medication therapy.
    • R1.1.2.  (Applying) Interact effectively with patients, family members, and caregivers.
    • R1.1.3.  (Analyzing) Collect information on which to base safe and effective medication therapy.
    • R1.1.4  (Analyzing) Analyze and assess information on which to base safe and effective medication therapy.
    • R1.1.5  (Creating) Design or redesign safe and effective patient-centered therapeutic regimens and monitoring plans (care plans).
    • R1.1.6  (Applying) Ensure implementation of therapeutic regimens and monitoring plans (care plans) by taking appropriate follow-up actions.
    • R1.1.7 (Applying) Document direct patient care activities appropriately in the medical record where appropriate.
    • R1.1.8  (Applying) Demonstrate responsibility to patients.
    • R1.3.1  (Applying) Prepare and dispense medication following best practices and the organization’s policies and procedures.
    • 2.1.2 Participate in a medication-use evaluation.

If the resident fails to meet any one of the above objectives in 12 months, they may be granted additional time to complete the residency, according to Remediation and Failure to Progress Process for Pharmacy 居民 Policy (RPH-D-766-004).

During the first three quarters, quarterly customized residency/resident development planning will include review of status of residents’ progression toward meeting each element required for completion. Formal check list is to be completed at the end of 4th quarter. See Appendix Residency Completion Checklist.

Residency Program Certificate

 

Upon successful completion of all program requirements and compliance with all conditions of the residency program the resident will receive a certificate indicating successful completion of the residency program.

 

居民 who fail to complete all program requirements and/or do not comply with all conditions of the residency program shall not be awarded a certificate of completion.
